Redbridge Stepdown provides support services to adults with substance misuse problems as they move on from residential rehabilitation services and return to the borough of Redbridge.

 

Redbridge Stepdown Service Aims

Redbridge Stepdown aims to improve the quality of life and opportunities for its clients that require resettlement support to move to independent accommodation. RSD aims to:

  • Increase the number of clients accessing stepdown / wraparound services and empower then to develop confidence and skill in managing their own needs and communicating effectively with agencies
  • Reduce relapse and the number of clients returning to drug related offending
  • Support clients to maximise and maintain income
  • Support clients to address issues arising from social and family relationships
  • Assist clients to obtain accommodation or sustain existing accommodation
  • Facilitate access to general and specialized health care and / or counseling
  • Enable clients to access appropriate education, employment and training opportunities
  • Enable clients to remain in treatment and develop longer-term coping strategies and life skills

Redbridge Stepdown Referral Criteria

This service is for:

  • People who are 18 or over and a resident of Redbridge (in any tenure including of no fixed abode, or in custody)
  • Substance misuse clients who are in treatment and have been identified as needing services by referring agencies.

Clients must have a demonstrable need for support related to one the indicators:

  • Substance dependency
  • Leaving treatment and returning to the community (could be residential or prison
  • Being resettled in appropriate housing
  • History of repeated and unplanned loss of accommodation
  • At risk of re-offending
  • Lack of social skills
  • In need of training employment

The service will priorities people leaving residential drug or alcohol services. Where capacity, we will also work with people who are in temporary accommodation and have support needs relating to substance use.

Referral to the service will be through:

  • Drug and alcohol workers in treatment services
  • Clients themselves attending the surgeries we will run at treatment agencies
  • Probation Officers or DIP workers

Redbridge Stepdown Service Delivery

The service is delivered through:

  • Flexible, one-to-one support: Following the initial assessment, an allocated worker visits with them to address their individual tenancy issues. The frequency of visits is determined by the person’s needs and circumstances.
  • Ongoing Support: Ongoing support is provided by the same allocated worker, usually in the client’s home.. Visits may be weekly, fortnightly or monthly according to the needs of each individual.
  • Assessment and Reviews: Each case is subject to regular reviews by the allocated worker and client. These reviews can also involve the service manager as and when required. Clients can also request a review at any time.

This service involves intensive work with individuals, families and carers who are receiving an intervention from a treatment agency.

Support provided may include:

  • Detailed support plans that are tailored to the needs of the client
  • At the point of resettlement, support clients into accommodation or provide advice and assistance to enable them to maintain their accommodation.
  • Access to employment, training, education, advice and referral.
  • Support to develop a wide range of living skills.
  • Support for clients to access services addressing literacy and numeracy.
  • Emotional and mental health support and referral to appropriate agencies.
  • Support to ensure physical health needs are properly met.
  • Support in addressing issues arising from social and family relationships.
  • Referral to appropriate legal advice.
